Donation Box

Form: Donation Box Application
Any placement of a donation box within the city limits of Houston must have a valid donation box permit.

Requirements: Applicants must:

  • Submit Donation Box Application
  • Submit site plan or map (for each donation box)
  • Submit service plan (for each donation box)
  • Pay appropriate fees (see schedule)
  • Submit and maintain commercial general liability insurance with coverage of not less than $1,000,000. The city shall be listed as an additional insured.

Permit expiration: One year from date of issuance
