Support and Coordination is an ongoing process that includes assessing
the needs of a caregiver and care recipient, effectively planning,
arranging, coordinating and following-up on services which most appropriately
meet the identified needs as mutually defined by the caregiver, the
care recipient and the access and assistance staff. Caregiver Education and Trainings, Support Groups, Workshops Many of our Network providers specialize in addressing the needs of Caregivers This includes developing a resource library, developing informational resources, developing and/or facilitating support groups, seminars and focus groups, facilitating individual or group counseling and providing educational services to groups, caregiver professionals or individuals. The Alzheimer’s Association provides qualified knowledgeable professionals skilled in presenting educational programs on Alzheimer’s and related disorders as well as holding Caregiver educational workshops and support groups. The Chinese Community Center conducts caregiver support groups as well as Caregiver Education and Trainings and workshops in Harris County, Texas. The Harris County Hospital District through Gateway To Care provides health promotion and education services throughout Harris County, Texas. These health promotion activities are designed to assist caregivers in making decisions and solving problems relating to care giving. The HCAAA Access and Assistance Program provides counseling services to Caregivers in Harris County, Texas. Counselors are on duty from 7:30 a.m. to 5:30 p.m. and during other times when appointments are scheduled in advance. Texas Southern University
Center on Aging conducts
caregiver support groups and provides information to support caregivers
who live with or work with older adults. Subject matter will include
signs and symptoms of elder mistreatment, risk factors for elder abuse,
caregiver burnout and stress management techniques, and caregiver
support and referral resources.
Instruction and Training Services Harris County Hospital
District through Gateway To Care
Provides medication misuse /substance abuse sessions specifically
related to the misuse of medication by older adults. This service
will include specific instruction/training designated to comply with
the Title III-D Medication Management, Screening & Education requirements.
Respite Aide Instruction and Training Services
The
Alzheimer’s Association provides
recreation and leisure activities such as sports, performing arts,
games and crafts in which older individuals participate as spectators
or Alzheimer’s Associations identifies performers. Alzheimer’s Association
recruits and trains interested participants and provide training which
includes respite aid training, caregiver neglect and abuse, caregiver
support groups for older caregivers, limited –English speaking caregivers
of older adults with dementia related disorders and low-income caregivers.
Assisting an older person that has difficulty performing a minimum of two activities of daily living identified in the assessment process, with tasks that an individual would typically perform if they were able. This covers hands-on assistance in all activities of daily living. Harris

and caregiver to determine the need for personal assistance.
If needed, an authorization is processed through
The HCAAA Access and Assistance Program. An array of services provided in a congregate, non-residential setting to dependent older persons who need supervision but do not require institutionalization. These services may include any combination of social or recreational activities, health maintenance, transportation, meals and other supportive services. Harris

and caregiver to determine the need for personal assistance.

Interfaith CarePartners delivers respite care services to frail older adults that will enable them to remain in their home and independent for as long as appropriate and experience impairment as less of a burden. This service also provides caregivers with a lifted burden of caring for a loved one by allowing for rest or freedom to attend to other matters that may be neglected due to the constant demands of care giving. Services are provided in the client’s home environment on a short-term, temporary basis while the primary caregiver is unavailable or needs relief. In addition to supervision, services may include meal preparation, housekeeping, assistance with personal care and/or social and recreational activities. Harris

The Asian American Family Center overall objectives is to enhance the mental health to Asian and non Asian older adults in Harris County Texas by providing educational seminars, support groups as a preventive measure, information and referral, as well as, intervention in the form of individual and group therapy.
The HCAAA Access and Assistance Program
provides counseling services to participants, in the
areas of crisis intervention, housing transitions, grief counseling,
and depression and anxiety, and stress management. Sheltering
Arms staff will check to ensure that the assessment of a client’s
problem was accurate, appropriate referrals were made, educational
materials were supplied to client and follow-up procedures were implemented.
A service provided by trained and supervised homemakers involving the performance of housekeeping/home management, meal preparation and/or escort tasks and shopping assistance, provided to individuals who require assistance with these activities in their place of residence. The objective is to help the recipient sustain independent living in a safe and healthful home environment. Harris
