The Nurse Family Partnership is an evidence-based, community health program  that helps transform the lives of vulnerable, low income women who are pregnant with their first child.

There are two locations serving North and South Houston.  Acres Homes (North) 832-393-4110 or Sunnyside (South) 832-395-0070.

The program is designed to:

  • Improve pregnancy outcomes by promoting positive health-related behaviors.
  • Improve child health, development, and safety by promoting competent care-giving.
  • Enhance parent life-course development by promoting pregnancy planning, educational achievement, and employment.


The secondary goals of the program are to enhance families’ material support by providing links with health and social services and to promote supportive relationships among family and friends. Each family partners with a well-trained registered nurse early in the mother’s pregnancy. The mother then receives ongoing nurse home visits through her child’s second birthday. Nurses adhere to developmentally-appropriate guidelines in assessing the child’s growth and development.
