City of Houston's Dental program provides comprehensive and preventive dental services for single persons ages 1 through 20 years for children (1) through 20 years whose family is enrolled in the Women Infant Child (WIC) Children’s Health Insurance Program (CHIP); and for Medicaid recipients ages 1 through 20 years. Single

persons ages 1-20 years and do not have Medicaid, CHIP or WIC would have to go through a dental eligibility process. In order to qualify for this program, you must complete an eligibility screening and provide current documentation of the following: Income (family), Residency, Dependents, Marital Status, Identification

The mission of Community Health Choice is to educate and nurture expectant mothers in disadvantage neighborhoods. This program provides education to expectant mothers on proper child care, nutrition education and health literacy.

The Senior Program provides daily meals, entertainment, health education classes, daily fitness classes, transportation and field trips for senior citizens in the community. NCI implements programs to meet the needs of all attending senior citizens by linking them to the in house agencies to provide them with assistance as needed.

Women, Infants and Children (WIC) is a special supplemental nutrition program which provides health care referrals, nutrition education and nutritious foods to pregnant, breastfeeding and postpartum women and children under five who have nutrition health-related problems.

This office provides birth certificates. Birth certificates are not public records and may only be issued to properly qualified applicants who satisfy identification requirements per state law. Money order for $23.00 per birth certificate applies. Checks not accepted.

The University Speech, Language, and Hearing Clinic provide evaluation, screening and treatment for children with speech, language and/or hearing problems or differences. Individual or small group therapy is provided on a weekly basis. Services are provided through the United Way.

Houston Center of Literacy an open access computer lab that provides usage for internet search, email, learn English, job search and resume creation at no cost. The Houston Center of Literacy works in partnership with other local agencies to promote self sufficient individuals in the work place and provides referrals for other

needed services.

 

EL CENTRO DE CORAZON FQHC

El Centro de Corazon is a community-based Federally Qualified Health Center (FQHC), which provides prenatal and postpartum care, well woman exams, ultrasounds, family planning and behavioral health to uninsured and underinsured residents of Houston’s East End. TARGET HUNGER/HOUSTON FOOD BANK

The commodity Supplemental Food Program is designed to improve the health and nutrition of senior citizens, pregnant women and postpartum mothers, infants and children. Participants receive one box of food once a month. Who is eligible? People over 60 years old and income eligible, women who are pregnant, breastfeeding,

under one year post partum, infants/children under six years old who do not receive WIC.

Harris Health Systems provides Financial Assistance based on first come first serve, limited space available. To apply you need to bring, Identification, proof of address, income ,household composition, and immigration status if applicable, proof of health coverage if applicable.
