2020-21 Health & Wellness Program: Win for Life


Employees, spouses and retirees under 65 are eligible to participate in the wellness program. Employees and covered spouses who participate earn a medical rate discount for their 2020-21 medical benefits. Retirees are ineligible to earn a discount, but will be entered to earn a gift card for their participation.

Wellness in Three Easy Steps

Complete the three steps below by the deadlines to earn up to $900 off your 2020-21 medical plan rate.

Employees and covered spouses: Visit your physician for your annual physical exam by January 31, 2020 to earn 10%. Obtain your biometric measurements from your annual preventive exam. Your biometric measurements must be from February 1, 2019 or later.

You need up-to-date measurements for your:

  • Blood pressure
  • Total and HDL cholesterol
  • Height
  • Weight
  • Waist circumference

STEP 2 - HEALTH ASSESSMENT

Employees and covered spouses: Use your up-to-date measurements to complete the online Health Assessment at mycigna.com between May 1, 2019 – March 13, 2020 to earn 10%.

TIP: Be sure to use biometrics from February 1, 2019 or later. Don’t choose answers like “I don’t know” for your biometrics and answer all the questions.


STEP 3 - WELLNESS ENGAGEMENT
You must complete this final step to earn a discount on your 2020-21 medical plan rate.

Employees and covered spouses: Complete one of the five wellness engagement options between March 9, 2019 – March 13, 2020.

Who is eligible to earn the discount?

Active employees and their covered spouses are eligible for the incentives. Retirees and dependents may participate in the program, but are not eligible to earn the medical rate discounts.

How do I earn the discount?

Earning the discount for active employees is easy. Just correctly complete your online health assessment by March 30, 2020, then complete one of four wellness engagement options by the corresponding due date, and you can earn the $600 medical rate discount for active employees.

How does my spouse earn a discount?

To earn the $300 discount, your covered spouse must correctly complete the online health assessment by November 30, 2019, as well as complete one of five available wellness options by March 13, 2020.

What is the maximum discount that I can earn?

Employees in the employee only or employee + children tier can earn a $600 annual medical rate discount. Employees earn an additional $300 discount if their covered spouse completes the health assessment and one wellness option. Employees in the employee + spouse or employee + family tier can earn up to a $900 annual medical rate discount.

If I don’t take the online health assessment, can I still participate in the Wellness program and earn my discount?

No. The online health assessment is a prerequisite to Wellness participation. If you do not complete the health assessment by March 30, 2020, you are not eligible to earn a medical rate discount.

I just started working at the City of Houston. Am I required to take the health assessment?

Employees with benefits effective on or after September 1, 2019, are exempt from taking the online health assessment, and will be eligible for the annual discounted rates.

Why do some of the dates go back before the program officially started?
We want to reward you for your proactive wellness activities, like getting your annual exam, so we’ve included dates that go back to February 3, 2019, for preventive care and May 1, 2019, for telephonic coaching.
Why does Option A have a different ending date?

Preventive care activities take longer to report, so those activities need to be completed by January 31, 2020, to be included.

How do I find out about dates for Onsite Health Education Programs?

Look for announcements for dates of the seminars and health programs coming to a location near you. All dates will be posted at houstonwellnessconnection.org.

How do I know when I have completed the requirements to earn a discount?

When you log in to MyCigna.com, you and your covered spouse will see a percentage completed toward your goal. Earn 100% by completing the health assessment (10%) and wellness engagement options (90%) by the deadlines to earn up to $900 discount on your 2020 medical plan rate.

Employees with a covered spouse on their plan will need to earn 200% (100% for you, the active employee, and 100% for your spouse). Allow at least 60-90 days for preventive care activities to be auto-reported. You must self-report dental or vision exams, Introductory Onsite Nutrition Coaching visit and EAP Coaching visit.
