2018-19 wellness program: Win for Life

Creating a healthy lifestyle can be like playing a board game. Go to the gym - advance two spaces forward. Pack a healthy lunch - move forward three. Forget to take your blood pressure medication - take two steps back.

The Health and Wellness team is here to help you succeed. We offer activities and options to educate and motivate you to move forward in life and help you make the best decisions for your health. So get on board and Win for Life.

This year, you’ll recognize a familiar wellness program. You still need to visit your doctor for biometric measurements, take a health assessment and complete a wellness engagement option. But there are enhancements to the Win for Life program designed to increase involvement and fit your 21st-century lifestyle.


Complete the health assessment and wellness engagement options by the deadlines to earn up to $900 off your 2019 medical plan rate.

STEP 1 - Biometric numbers
You must complete this step before you can move on.

Employees and covered spouses: Obtain your biometric measurements from your annual preventive exam. Employees may participate in one of the limited biometric screening events across the city. Covered spouses must go to the doctor. Your biometric measurements must be from February 3, 2018, or later.

You need up-to-date measurements for your:

  • Blood pressureTotal and HDL cholesterol
  • Height
  • Weight
  • Waist circumference

You can register now for onsite biometric screening. Go online at or call 877-366-7483.

STEP 2 - Online health assessment
You must complete this step before you can move on.

Employees and covered spouses: Use your up-to-date measurements to complete the online health assessment at by December 31, 2018.

TIP: Be sure to use biometrics from February 3, 2018 or later. Don’t choose answers like “I don’t know” for your biometrics and answer all the questions.

STEP 3 - Wellness engagement
You must complete this final step to earn a discount on your 2019 medical plan rate.

Employees and covered spouses: Complete one of the wellness engagement option between February 3, 2018 and March 8, 2019.

  • Option A: Preventive Care, Deadline: January 31, 2019
  • Option B: Telephonic Coaching, Deadline: March 8, 2019
  • Option C: Onsite Lifestyle Management Programs (Employees only), Deadline: March 8, 2019
  • Option D: Fitness Tracker (Cigna Apps and Activities), Deadline: March 8, 2019
