Making your donation

City of Houston employees wishing to make a contribution through the 2008 CMC will be able to sign-up using an electronic pledge form available at the Employee Self Service application during October 2007.

If you want to fill out a paper form see your CMC Coordinator.

Log-on to ESS. First time users will need to complete the registration process for ESS.

1. Click on CMC Contribution Sign-up under the Combined Municipal Campaign menu on the lower left-hand side of the screen.

2. Fill in agency codes for your charities. If you don’t know the codes, see the Agency List or click Search and Select Agencies to make your choices.

3. Enter in the amount you wish to give to each agency. If you would like to be an Above and Beyond contributor, your total per paycheck amount must be equal to or greater than the 1 percent amount shown at the top of your form.

4. Choose a one-time or per paycheck deduction. This will apply to all agencies.

5. Select an option for a tax receipt. If you choose yes, you are authorizing the city to release your home address to your agencies.

6. Click Verify Selections to check your form. The agency name and totals will now appear. If there are any errors, a message in red will appear at the bottom of the form.

7. After you have looked over your form for accuracy and corrected any errors, click Continue to Confirmation Page.

8. The confirmation page shows summary of your selections, per paycheck and yearly totals, and displays your status as an Above and Beyond contributor. Review the information carefully. If you need to make corrections, click Edit Selections. If everything is correct, you must check the box next to “I authorize the City of Houston to deduct my CMC contributions,” and then click Authorize Deductions to complete the contribution process. If you need to make changes after this step, contact your CMC coordinator.
