16. Are pre-natal vitamins reimbursable? Do I need to complete the Medical Necessity form?

If you are pregnant and take pre-natal vitamins, they are eligible for reimbursement. You do not need to complete the Medical Necessity form.

17. If I take a calcium supplement, recommended by my doctor due to severe osteoporosis, is it reimbursable? Do I need to complete the Medical Necessity form?

Yes. Ask your doctor to complete the Letter of Medical Necessity, and submit the form with your first claim.

18. Are Glucosamine and Chondroitin reimbursable with a Medical Necessity form?

Yes, they are reimbursable if they are prescribed by your doctor to treat a specific medical condition. A Medical Necessity form is required.

19. Are eye drops (Visine) eligible?

Yes.

20. My doctor says that I should check my blood pressure regularly (no specific diagnosis). I buy a $150 home-blood-pressure-machine. Is it reimbursable?

As long as the receipt specifically indicates it is for blood pressure, it is eligible.

21. My doctor told me to take a daily baby aspirin for my heart. Do I need to complete the Medical Necessity form?

No. This is reimbursable under the HFSA as over-the-counter medication.

22. I have an AFLAC cancer policy. It has a $50 wellness benefit. I pay $80 for my annual physical. Can I submit the $80 to the HFSA for reimbursement after I receive the $50 back from AFLAC? What is reimbursed: $80 or $30?

Yes, you may submit the expense for your physical. $30 is reimbursable in this example.

23. I pay for a "qualified" long term care policy. Are those premiums eligible under this HFSA?

No. Insurance premiums are not eligible under HFSA.

24. Under what conditions is massage therapy reimbursable? Does it always require the Medical Necessity statement?

Generally, massage therapy is not eligible unless it is recommended by a physician to treat a specific injury or trauma. The cost of a massage just to improve general health is not reimbursable. To apply for reimbursement, submit a Medical Necessity form.

25. Are chiropractic services eligible for reimbursement?

Yes, chiropractic services are eligible, and a Medical Necessity letter is not required.

26. I have had special tubes inserted in my ears and must keep water out of them to prevent infection. My doctor has suggested I get custom-made ear plugs. Will these ear plugs be reimbursed?

Yes, with a Medical Necessity form.

27. Are prescriptions and receipts from other countries reimbursable, if the essential information is on the receipt?

Yes, as long as all the information is on the receipt, it is an FDA approved drug, and is in US dollars.

28. Is sales tax reimbursable for eligibile over-the-counter expenses?

Yes, the sales tax is reimbursable as part of the cost of the product.

29. If my doctor completes a Medical Necessity form this year, does he have to complete another form next year for the samehealth condition?

Yes, Medical Necessity forms must be filed every year, just as you must re-enroll in the FSA every year.

30. My doctor has recommended I join a health club so I can swim and use specific exercise equipment to relieve symptoms of a pinched nerve in my back. Is any of the health club membership/dues reimbursable?

No.

31. My doctor recommended walking as part of physical therapy. Is the cost of walking shoes reimbursable?

Athletic shoes are not reimbursable, but specially made orthopedic shoes are reimbursable.

32. I order my medicine through the Prime Therapeutics Mail Order Drug Plan. When is my prescription reimbursed?

You can file a claim for reimbursement once you have received your medication. The reimbursement is based on the date on the receipt/packing-slip. Just submit a copy of the packing slip with your claim form.

33. My spouse and I are employed by the city and we are both enrolled in the HFSAs. Can I submit my claims on her account?

Claims can only be submitted for reimbursement once, whether under your account or your spouse's.

34. Are expenses from unapproved dental treatment eligible for reimbursement under the HFSA?

Out-of-pocket expenses, deductibles, copayments, and coinsurance not paid by another plan are eligible for reimbursement. However, cosmetic procedures such as teeth bleaching are not eligible.

35. I made a $500 contribution to my HFSA, is that all I will be reimbursed?

You will be reimbursed up to the amount you elected to contribute.

36. I have 2 very active boys who are constantly needing glasses repair. How often can I submit repair costs for those eyeglasses?

You may submit eligible expenses as often as you incur them. However, your reimbursement will not exceed the amount you elected to contribute to your HFSA.

37. Can receipts be combined to total the minimum claim amount of $10.00?

Yes, you may combine receipts to equal the $10.00 minimum claim allowance.

38. What happens if an employee is laid off while participating in the HFSA?

An employee who is laid off while participating in the HFSA has 90 days after the date of layoff to submit claims that were incurred prior to the date of layoff up to the maximum contribution elected. If the employee elects COBRA coverage, he/she may submit claims so long as they are on COBRA benefits up to the maximum contribution amount elected.

39. Are expenses for over-the-counter methods of birth control reimbursable under the HFSA?

All legally approved birth control items are reimbursable including birth-control bills, condoms and spermicidal foam.

40. How does the HFSA work when one goes on unpaid leave during the plan year?

You may continue to submit claims up to the maximum amount you elected to contribute to your HFSA.
