    Diabetes self-management class
Free for city employees, retirees and dependents with diabetes
      

If you are diabetic, you need to attend this class on self-management.

Certified Diabetic Services, a specialty company contracted by HMO Blue Texas, will present a daylong workshop during which certified diabetic educators will demonstrate self-management strategies for minimizing diabetic-related complications and maintaining a better quality of life.

It’s free for HMO and PPO members with diabetes, their eligible dependents and guests.*

You’ll learn the latest strategies for:

• Better glucose self-management
• Basic lifestyle changes
• Modern diabetes medications
• Prevention and treatment of complications
• Meal planning, nutrition, dieting and cooking tips
• Exercising

You’ll receive:

• A free, accurate, easy-to-use glucose monitor valued at $60
• A personal-care coach to encourage good self-management practices
• A 90-day quantity of diabetic supplies delivered to your home. Copayment will apply
• Free telephone and e-mail access to CDS educators, registered nurses and dieticians
• Invitations to special diabetes community events

And this is the best part: All attendees will enjoy a delicious diabetes-friendly free lunch prepared by chef Scott Campbell, who is diabetic.
