Take the easy-to-do Mayo Clinic health risk assessment, authorize its release to BlueCross BlueShield of Texas and you win a $50 gift certificate to Academy if you live in Texas, or Sports Authority if you live outside of Texas. Of course, you’re really winning a bigger prize: guideposts to making better health and lifestyle choices.

Taking the HRA
The assessment takes about 15 minutes to complete. When you’re finished, experts calculate your health status and give it back to you in easy-to-understand language. And they give you ways to make improvements. Print it out for future reference.

To Play:
Before starting, it’s a good idea to collect your vital numbers: height, weight, blood pressure and also cholesterol numbers, triglycerides and blood sugar levels. The more accurate your information, the more accurate your assessment. But don’t worry if you don’t have this information, you can still play.

Rules of the Game:
Employees and retirees who are in the city’s HMO and PPO are eligible. Dependents are encouraged to complete an HRA, but only one gift certificate will be issued per family, employee or retiree. After the employee or retiree completes the HRA, a gift certificate will be mailed to the employee or retiree’s home address within 60 - 90 days. To be eligible, you must authorize release of your HRA to BCBSTX by clicking the appropriate box. And don’t worry about privacy. The Mayo Clinic will keep your information confidential. If you have already taken the HRA this calendar year and authorized its release, you will get your certificate at the end of July. Otherwise, you will need to retake the HRA and authorize its release to qualify.

How do I get started?

  1. Log on to www.bcbstx.com and Blue Access for Members. If you haven't already registered, do so now by clicking on New User Sign-up.
  2. Scroll down and click on Personal Health Manager. It's the blue logo with three silhouettes in the right-hand corner.
  3. Look for the Mayo Clinic logo near the center of the page and click on Take the Health Risk Assessment.
  4. Follow the HRA instructions and complete the survey.
  5. Check the Authorization to Release box when finished.
  6. Look for your gift card in the mail within 60-90 days.
