Find a dental provider online: It is easy to access a United Healthcare/National Pacific dental provider online for the DHMO and the dental indemnity plan. Visit www.yourdentalplan.com/npd to locate a dentist. For the DHMO, you must have a primary care dentist provide referrals for specialist dental care. You may change your dentist monthly by calling the United Healthcare/National Pacific Customer Service, (866) 605-2599.

AFLAC wellness benefits
Complete a claim form, write “wellness” on your receipts and fax to (877) 442-3522. For questions, call AFLAC (281) 448-2240.

  • The Personal Cancer Protection Plan will reimburse each covered family member an annual $75 wellness benefit for covered cancer screenings, including prostate screening, chest X-rays, mammograms and PAP smears.
  • The Accident/Disability Policy will reimburse a covered family member an annual $60 wellness benefit for wellness screenings, including prostate screening, chest X-rays, mammograms, PAP smears and physical examinations. Employees must have been covered for 12 months.
  • The Hospital Indemnity Policy will reimburse $50 annually for a covered family member’s routine examinations or preventive testing, including dental and eye exams, immunizations, physical exam, mammograms, Pap smears, etc. This benefit requires coverage for 12 months.

It’s not too late to get your free $50 gift card
Log onto www.bcbstx.com, click on the Personal Health Manager and click on the Mayo Clinic to take the Health Risk Assessment. It is quick, easy and takes about 15 minutes! You will receive a $50 gift card just for completing and releasing it to BlueCross BlueShield. Look for your gift card in the mail during the month after you take the HRA. Limit one gift card per family.

Diabetes self-management class
All employees, retirees and dependents who are diabetic* are invited to attend a free diabetes class presented by Certified Diabetic Services.

Learn how you can control diabetes. Instructors will answer all your questions regarding weight management, diet, exercise, cooking, glucose monitoring, medications and more.

Attendees will receive a free diabetic friendly lunch, a glucose monitor, and invitations to future CDS diabetes events.

The class will be held at the E.B. Cape center, 4501 Leeland, Houston, TX 77023 on Sat. Nov. 4 from 8 a.m. to 5 p.m. Attendees must pre-register by calling John at (888) 966-6909.

*Kelsey-Seybold members are required to attend Kelsey-Seybold sponsored diabetes classes and are not eligible for this class. Contact Kelsey-Seybold to find out more information about their diabetes classes.

Dependent care enrollment starts soon
Do you have dependent care expenses for children under 13 or dependent expenses for someone over 13? Do you want a tax break?
Employees eligible for city benefits can take advantage of the Dependent Care Reimbursement Plan. Lower your taxable payroll income by setting aside pretax dollars to pay for qualifying expenses.

Have a shot at beating the flu
Flu season usually starts in November and peaks in late December or January. The Centers for Disease Control and Prevention recommend an annual vaccination. It takes two weeks for the vaccine to become fully effective so the ideal time for vaccination is October and early November.

The CDC strongly recommends a flu vaccine if you:
Are 65 or older

  • Are a resident in a chronic-care facility
  • Have asthma, emphysema or another respiratory disorder
  • Have heart disease, diabetes or another chronic health problem
  • Have a suppressed immune system

The CDC recommends a vaccine for:

  • Healthy children 6 to 23 months
  • Otherwise healthy adults 50 to 64
  • Pregnant women in their second or third trimester
  • Health care /EMS workers
  • A child or teenager receiving long-term aspirin therapy

The CDC advises a vaccine for:

  • Anyone without a previous allergic reaction to the flu vaccine or a severe allergic reaction to eggs

If you miss the shot, the antiviral medications Relenza, which is inhaled, and Tamiflu, which comes in a tablet or liquid, cut the odds of getting sick by as much as 70 to 90 percent. Studies show these drugs provide a 29 to 43 percent reduction in complications if taken within 48 hours of the onset of symptoms.

Save with dependent care

Save money on dependent care for eligible dependents for the plan year Jan. 1 – Dec. 31, 2007.

Employees eligible for city benefits are eligible to direct pretax dollars into this plan to reimburse out-of-pocket dependent care expenses for:

• Children under age 13 you claim as dependents for tax purposes.
– Licensed nursery school and
children’s day-care centers.
– Food and education provided as part of preschool care.

• Spouse or dependents regardless of age who are mentally or physically disabled and who regularly spend at least eight hours a day in your home if services are rendered outside the home.
– Licensed day-care centers for
disabled dependents.
