DATA2CARE - BENEFITS FOR PROVIDERS
BENEFITS FOR THE PROVIDER:
Quarterly outcome reports on the out-of-care patients referred by providers to the Houston Health Department (HHD)
HHD staff will review epidemiological data and go into the community to locate patients.
HHD will work to refer patients back to your office.
HHD will link patients to other services based on their needs such as housing, food, transportation, financial assistance, etc.
BENEFIT TO THE COMMUNITY:
Reduction in the risk of new HIV infections
Improved health outcomes
STEPS IN THE PROCESS:
1. POINT-OF-ENTRY AGREEMENT:
Provider signs a point-of-entry agreement with HHD
2. LIST GENERATION:
Provider generates a list of out-of-care patients to share with HHD
3. SURVEILLANCE INVESTIGATION:
HHD’s Surveillance Investigators (SI) look for care status of referred patients through epidemiological data.
4. SERVICE LINKAGE:
HHD’s Service Linkage (SL) Workers contact patients who are confirmed to be out of care by the surveillance investigators (SI) and assist with re-linking them to services.
5. INFORMATION SHARING:
Patient status of care is compiled to share with the provider.
