BENEFITS FOR THE PROVIDER:

  • Quarterly outcome reports on the out-of-care patients referred by providers to the Houston Health Department (HHD)
  • HHD staff will review epidemiological data and go into the community to locate patients.
  • HHD will work to refer patients back to your office.
  • HHD will link patients to other services based on their needs such as housing, food, transportation, financial assistance, etc.

  • BENEFIT TO THE COMMUNITY:

  • Reduction in the risk of new HIV infections
  • Improved health outcomes

  • STEPS IN THE PROCESS:

    1. POINT-OF-ENTRY AGREEMENT:

  • Provider signs a point-of-entry agreement with HHD


  • 2. LIST GENERATION:

  • Provider generates a list of out-of-care patients to share with HHD


  • 3. SURVEILLANCE INVESTIGATION:

  • HHD’s Surveillance Investigators (SI) look for care status of referred patients through epidemiological data.


  • 4. SERVICE LINKAGE:

  • HHD’s Service Linkage (SL) Workers contact patients who are confirmed to be out of care by the surveillance investigators (SI) and assist with re-linking them to services.


  • 5. INFORMATION SHARING:

  • Patient status of care is compiled to share with the provider.

  •
