Active Employees and Retirees Under 65 earn chances to win a $250 gift card* based upon their Health Assessment completion date.
*Employees and Retirees must earn 100% within the Win for Life Program in order to be eligible to win a $250 gift card.
Active Employees and Retirees Under 65 will earn Healthy Awards Account dollars** based upon their Health Assessment completion date.
Healthy Awards Account dollars may be utilized for medical and pharmacy expenses.

Active employees who complete the health assessment early will be entered into a drawing to receive a prize based upon their date of completion. Employees who complete the health assessment earlier will have a chance to win both prizes.
Employees within the top three departments (by tier) with the highest overall percentage of Health Assessment completions will be entered into a monthly drawing for a $100 gift card.
The department tiers are as follows:
- Level A - Departments with 1,500 + employees: (Fire, Police,Houston Public Works)
- Level B - Departments with 500 to 1,499 employees: (Aviation, Houston Health Department, Parks
- Level C - Departments with up to 499 employees: (All others)
