EARLY BIRD INCENTIVES

The earlier you complete your Health Assessment, the better your chances are for more rewards! You have multiple opportunities to earn gift cards, prizes and Healthy Award funds! Active employees and covered spouses can earn chances to win a new Fitbit for completing the Health Assessment, based on their completion date. Active employees who complete the Health Assessment early receive more chances to win prizes based on their completion date.

DEPARTMENT CHALLENGE

Employees within the top three departments (by tier) with the highest overall percentage of Health Assessment completions will be entered into a monthly drawing for a $50 gift card.
