Employees

Your enrollment forms are due in your department by the date established by your department. Look for communication from your department regarding meetings and due dates.

The final date to submit enrollment forms is noon, Friday, April 8.

Retirees

Your enrollment forms are due by noon, Friday, April 8.

Retirees will submit enrollment forms in the stamped envelope to the Benefits Division at 611 Walker, 4th floor.

Medicare Advantage Plans

If you meet eligibility requirements, you may join now, or on the 1st of any month.

  1. Forms are due to the city by April 30, 2005 for coverage to begin May 1, 2005.
  2. Forms received after Apr. 30, 2005, will become effective on the first day of the following month.
  3. To pay the lower premium the benefits division must receive your application by the third of the month prior to the month that you want coverage to start. If you want to pay the lower premium on May 1, the benefits division should receive your form by April 3. If we don’t, you will receive a refund for the higher premuim for May 1.

We urge you to consider participating in one of these new money-saving managed care plans.

You have the option for split-family elections, and in 2005, you can switch between plans on the first of any month. In 2006, you can return to the HMO or PPO on January 1 or May 1. Try it ... you'll like it!! Click here for information on how to enroll and disenroll.

Important note:
If you are enrolled in the medical, dental or supplemental insurance plans and don’t want to make any changes - don’t do anything. Your coverage will remain in effect through April 30, 2006.

Important note: For medical and dental plans, only new enrollees will receive new ID cards for May 1, 2005. Your current ID card has no expiration date. If you need additional or replacement ID cards, order them through the provider, as usual.
