Points of Consideration

bullet Deadline claims can be filed: March 31, 2009 – 90 days after plan year, Jan. 1 - Dec. 31, 2008, ends
bullet Family status changes can result in a change of an employee’s enrollment.
bullet Minimum contribution amount = $99.84 per year
bullet Maximum contribution amount = $5,000 per year ($2,500 if married and filing separate tax returns). If you and your spouse both work for the city, you may each contribute up to $2,500 per year.
bullet You must have at least a $15 expense to submit a reimbursement claim.
bullet File a claim after employee termination, if the expense was incurred before termination.
bullet Reimbursement is limited to actual contributions.
