Actions: Complete at least two preventive care activities. You will earn credit for activities between February 1, 2017, and February 2, 2018. All medical services must be with a Cigna healthcare professional and will be automatically reported through a claims submission, except where noted as self-reported.

Option B: Telephonic Coaching
Available: May 1, 2016 – March 9, 2018
Actions: Work your way toward a health goal by speaking with a health coach over the phone. Coaches can counsel you in weight loss, smoking cessation, medication adherence, disease management and many other health concerns. This option is not complete until the health coach approves your goal achievement. Call 800-997-1406 to start this option today. Don’t wait until the last minute.

Option C: Onsite Programs (Available for employees only)
Available: August 1, 2017 – March 9, 2018
Actions: Attend a weekly Onsite Lifestyle Management Program for six weeks. Topics include diabetes prevention/management, cardiovascular disease prevention/management and general prevention. Visit houstonwellnessconnection.org for a schedule of classes near you starting in August 2017.

Option D: Fitness Tracker (Cigna Apps and Activities)
Available: August 1, 2017 - March 9, 2018
Actions: Track physical activity, diet and health goals using a fitness tracker device such as a Fitbit or Up by Jawbone, or app such as My Fitness Pal, Map My Fitness, RunKeeper and more. Apps and devices from an approved list can be integrated with Cigna’s Apps & Activities portal to complete various goals and challenges such as tracking your sleep, taking 10,000 steps or testing your blood glucose. Each goal and challenge earns varying amounts of points depending on the level of difficulty. You must earn a total of 1,000 points.
